JCAHO requires health-care organizations to perform at least one FMEA (Failure Mode Effects Analysis) each year. This book is a guide on how to perform an FMEA with fault trees. Examples are illustrated with medical errors reported in the literature. The included software (requires Windows 2000 or later) facilitates FMEA preparation.
