The Triage Form along with The Financial Statements and The Buy and Sell Forms are the combination of all of the number crunching (analysis of the ratios and formulas) along with their respective forms (from the financial statements). The Triage Form aids in valuing, tracking and keeping up with the stocks that you have chosen to add to your portfolio from beginning to end.

The information, of course comes from the three volumes that I also recently published under the title series The P/E Logo: The Motivation To Invest The Motivation To Win:

1) The Motivation To Invest Fundamental Valuation Volume I
2) The Motivation To Invest Key Statistics Volume II
3) The Motivation To Invest Technical Analysis and Portfolio Management Volume III

Each volume covers the three different aspects of valuing a stock, which comes from putting together the numbers from the financial statements: The Balance Sheet, The Income Statement and The Cash Flow Statement. This is where one would go to find out how a company is doing and thus the stocks that they are offering also.

Each of the above books add background of how I got started on this venture of trying to figure out the best route to take when trying to put together a personal investing portfolio. I add events from secondary school to college and a lot of the places in between that somewhat guided me along this path, which I felt people would find interesting. Many will find themselves associating with some if not most of the events which happed to me that led me to pursuing the course of personal investing for myself and in aiding other who may be on that same track.

I then knew that there would be a time, besides the enjoying of reading about that venture, that I would want to as well as need to grab a copy of just the Triage Form or one of the financial Statements, and quickly try and interpret and make since out of the key statistics when I would need to make, at that moment, an important decision on either buying a stock or selling one. With all of the IPOs (Initial Public Offerings), M & A (Mergers and Acquisitions) and E. R. (Earnings Reports) and the many other announcements (jobs reports, employment reports, housing, GDP (domestic and foreign), where having this information readily available would be somewhat vital. Each item on the above list affects the market and thus the stocks that you have chosen or are thinking of choosing.
After reading the first book (The Motivation To Invest The Motivation To Win: Fundamental Valuation on How To Buy and Sell Stocks), the Triage Form and Definitions will make more sense to those that view it when beginning to literally pick a stock or create a portfolio to invest their hard earned money into. After understanding it more, The Triage Form can be added to a growing list of formulas and terms that you may run across, and still be as easy to learn as the initial one. Once getting the feel of it, things become clearer and easier, and as you start to learn more, you’ll also start to add more ways of valuing a stock to that list.
Whether your list for valuing stocks comes out to a page long or twenty pages long, it doesn't matter because there are several ways of valuing a stock and thus there can be several lists with different fundamental valuation or technical analysis methods used to get different perspectives on a stock. I started off first with the most used methods by most investors and worked out from there.

The fundamentals or rather the Fundamental Valuation methodology is the interpretation of the numbers like:
Gross Revenue
Gross Profit
Gross Margin
Operating Expense
Or
Operating Income
Net Income Applicable to Common Shares
Etc.
