It contains four medical scenarios with questions for both the patient and proxy. The scenarios are designed to foster dialogue between patients and proxies in order to define possible choices for care and to increase patient/proxy understanding and knowledge.
The short companion video introduces the topic of planning ahead for end-of-life decisions and portrays a compelling, unrehearsed real-life conversation between actress Kathleen Chalfont,("Wit", "Picket Fences") and her husband, filmmaker Henry Chalfont, based on several of the scenarios from the workbook.
