Fidelity, Wisdom and Love is an educational program designed by faculty of the Division of Medical Ethics at Weill Medical College of Cornell University. It consists of a workbook and video designed to help people plan ahead for their care at the end of life.

The interactive workbook is a step-by-step guide that provides everything you need to know about choosing, appointing, or being a healthcare proxy. It contains four medical scenarios with questions for both the patient and proxy. The scenarios are designed to foster dialogue between patients and proxies in order to define possible choices for care and to increase patient/proxy understanding and knowledge.

The short companion video introduces the topic of planning ahead for end-of-life decisions and portrays a compelling, unrehearsed real-life conversation between actress Kathleen Chalfont,("Wit", "Picket Fences") and her husband, filmmaker Henry Chalfont, based on several of the scenarios from the workbook.
