From the author: Managed care is considered by most physicians to be the major culprit in what ails America's health system. Possibly the overriding conflict, however, relates to an attempt to restructure the health system by employing business principles to the delivery of medical care services. After more than a decade of unprecedented properity, an almost inevitable sharp economic downturn--one that causes health networks to divest themselves of unprofitable physician practices and to shutter the more unproductive programs, services. and facilities--will cause even greater physician discontent. Public officials recognize the political difficulties in enacting any health reform measures that would guarantee universal access and social equity. But they could opt for a global budget process that would keep most of the market-driven competitive forces in tact and, yet, would lessen the customary rise in health expenditures. Both physician executives and community-based practitioners will encounter an increasingly complex and c ontentious environment, one in which coping with change is the norm.
